Awabakal Aged Care Services is a comprehensive program that offers quality services to our community.
Awabakal Aged Care Services staff will work with you to develop what is known as ‘My Story’. This will include asking questions about your family, your history and your health.
This is where you are able to discuss what is important to you, any worries you have about your health, what you might need to help improve your day to day life and what kind of support you require.
Based on your story, Awabakal will help with building a ‘plan’ for you. This plan will be tailored to your individual needs and will incorporate the services you will require.
Our Aged Care Team is dedicated to providing culturally safe, person-centred support to Aboriginal Elders and community members. We work closely with clients and their families to promote independence, wellbeing, and connection to culture.
Our core services include care coordination, home care support, advocacy, transport assistance, social connection, and guidance navigating My Aged Care and the Support at Home Program. We collaborate with clinical and community teams to ensure each client receives holistic, tailored care that meets their individual needs and goals.
With a strong commitment to enhancing quality of life, our team walks alongside Elders to ensure they feel supported, respected, and empowered every step of the way.
The program supports our community to live independently at home and in our community.
Support services – such as help with basic home maintenance and everyday tasks, home modifications related to your care needs and transport services.
Personal care – such as help with showering or bathing, dressing and mobility.
Nursing, allied health and other clinical services – such as physiotherapy, occupational therapy and remedial massage.
Care coordination and case management.
Light housework, meal delivery, welfare check-ins. For largely independent individuals.
Assistance with dressing, showering, medications, and social activities.
Regular personal care, mobility aid support, meal prep, household cleaning.
Daily routines assistance, home modifications, toileting, continence care.
Hoists, transfers, transport to medical appointments, in-home safety monitoring.
Nursing support (wound care, medication), continence management, allied health therapies.
Assistance with all daily living activities, complex health conditions, dementia support.
Palliative support, 24/7 availability, specialised equipment, nursing interventions.

Awabakal’s Medical Service is well placed to assist our ageing community with their healthcare needs.
The Medical Service is staffed by General Practitioners, Aboriginal Health Workers, Nurses, Specialists and Allied Health Professionals.
We specialise in chronic disease management. We can also assist you with medication management. In addition, we run a series of programs, activities and community events throughout the year.
